What Community Care Connection provides, what it does not, and the ground rules for using it.

The service
Community Care Connection provides care navigation and resource connection. We help you understand options, identify organizations that may be able to help, send referrals with your consent, and follow up.
We do not provide medical, nursing, therapy, legal or financial services. We do not supervise or control the organizations in our network.
No clinical or eligibility decisions
Nothing on this site is medical advice. We do not diagnose conditions, recommend treatment or medication, interpret test results, or determine your eligibility for any program or benefit. Only the relevant clinician, agency or program can do those things.
CCC VERIFIED™
CCC VERIFIED™ means our team reviewed an organization's records on the date shown next to the badge. It is not an endorsement, certification, accreditation or inspection.
It does not guarantee clinical quality, safety, licensure status at any later date, availability, outcomes, or that the organization is suitable for your situation. You are responsible for your own decision about any organization you contact.
No referral fees
We do not pay or accept fees for referrals. We do not rank organizations by payment. If any listing is ever sponsored, it will be clearly labeled as sponsored placement.
Using the service
When you use Community Care Connection, you agree to:
- Share accurate information, and only the information needed for navigation.
- Not use the service for an emergency — call 911 instead.
- Not use the service to harass, mislead or attempt to access another person's information.
- Respect that providers decide for themselves whether they can accept a referral.
Accounts
You are responsible for keeping your account credentials secure. Provider and navigator accounts carry additional responsibilities, including keeping availability and referral responses current.
Limitation
The service is provided as-is. We do not guarantee that an organization will accept a referral, that a connection will succeed, or that listed information is complete or current. To the extent permitted by law, Community Care Connection is not liable for decisions made by you, by a provider, or by any third-party organization.
Changes
We may update these terms as the service grows. Material changes will be reflected in the date at the top of this page.
