Understand
You tell Aunty — or a person — what is happening, in your own words. No forms full of jargon, no eligibility quiz.

Navigate
We turn that into your Care Connection Map™: what we heard, the kinds of support that may fit, possible next steps, and community resources.

Match
A care navigator reviews your situation and identifies organizations that may be able to help, based on service, island, availability and your preferences.
04
Connect
With your consent, we send a referral. The organization accepts, asks for more information, or declines — and you can see where things stand.
05
Follow up
We check back. If a connection did not work out, that is not the end of the conversation — your navigator keeps going.
What Aunty can and cannot do
Aunty can

Aunty cannot
- Diagnose or interpret test results
- Advise on medication or treatment
- Decide program eligibility
- Make emergency decisions
- Guarantee any provider or outcome

Your information
Your Aunty conversations and Care Connection Maps are saved to your account so you do not have to repeat your story. You can read them any time.
We share your information with a provider only after you ask us to connect you, and only what is needed to make that connection possible.
Community Care Connection is a care-navigation service. We are not a HIPAA-certified covered entity and we never claim to be.
