Aunty™

Start by telling Aunty what's going on.

No forms to decode, no jargon. Aunty asks a few questions the way a trusted aunty would, then turns your answers into a plan you can actually use.

You'll be asked to sign in so your conversation and your Care Connection Map are saved to your account.

What Aunty can do

  • Listen to what's happening in your own words
  • Help you name the kinds of support that may be relevant
  • Point you to community resources and programs
  • Build a Care Connection Map™ you can keep and share
  • Hand you to a human navigator whenever you want one

What Aunty will never do

  • Diagnose a condition or tell you what's wrong
  • Advise on medication, treatment or test results
  • Decide whether you qualify for a program or benefit
  • Make emergency or clinical decisions
  • Replace your doctor, nurse, social worker or 911

“I can help you navigate resources, but I can't diagnose or provide medical treatment advice.”

IF THIS IS AN EMERGENCY, CALL 911.

No Wrong Door™

Sometimes another organization is the better place to start — and that is a good outcome. If Community Care Connection is not the right fit, we will say so and help you identify where to go instead.
