Why we exist
Care in Hawaiʻi is not short on good organizations. It is short on clear starting points. Families tell us the same thing over and over: they made eleven phone calls, filled out four forms, and still did not know what to do next.
We built Community Care Connection to hold that confusion for a moment so a family does not have to. You tell us what is happening in your own words. We help you see the kinds of support that may fit, point you toward community resources, and — when you want it — a care navigator reaches out and helps you connect.
You Don't Have to Navigate This Alone.™
What we are not
- Not a medical provider. We do not diagnose, treat, or advise on medications.
- Not a replacement for your physician, hospital, licensed case manager, or 911.
- Not a plain directory — a directory is a list; navigation is a relationship.
- Not an AI chatbot pretending to be a person. Aunty is a guide, and humans do the rest.
- Not paid by providers for placement or referrals.
