Hospice families are carrying grief and a to-do list at the same time. We help with the second one so your team can stay with the first.

What we hear from hospice teams

  • Caregivers who have not slept properly in weeks and will not ask for respite.
  • Families needing practical help — meals, transportation, equipment logistics, extra hands.
  • ʻOhana members arriving from off-island with no idea how to help.
  • Bereaved caregivers who lose their support network the week after.

Boundaries

  • We do not provide hospice, palliative, nursing or bereavement clinical care.
  • We do not advise on symptom management, medication or goals of care.
  • We do not make decisions on behalf of a patient or family.
  • We do not pay or accept referral fees.

Community Care Connection is a care-navigation and resource-connection service. We are not a covered entity, we do not claim HIPAA certification, and we do not replace clinical judgment or discharge responsibility.

What we do for your team

Connect families to respite and caregiver support in their community.
Navigate volunteer, meal, transportation and practical-support resources.
Give families a written summary so every ʻohana member sees the same plan.
Coordinate with the family's chosen organizations, not around them.
Keep checking in, including after a loss, when the family wants that.
Refer outward to grief and community support that we do not provide.

Available today: referrals in, navigator follow-up, and outcome reporting by email. These are planned, not live yet:
